Our goal is to promote independence, dignity, and safety by delivering quality care that’s personalized to each client’s unique needs.

CFSS Services – Assistance with Activities of Daily Living (ADLs)
At Maxx Home Health Care LLC, our Consumer-Directed Community First Services and Supports (CFSS) help individuals maintain independence while receiving personalized support at home. We operate under the agency model, meaning we hire and train employees to provide care, but clients also have the option to bring their own employee if they prefer.
CFSS covers assistance with Activities of Daily Living (ADLs), including:
- Personal Care: Bathing, grooming, dressing, toileting, and hygiene support
- Mobility Assistance: Helping with transfers, walking, and safe movement around the home
- Meal Assistance: Preparing and supporting proper nutrition
- Medication Reminders: Ensuring medications are taken safely and on time
- Companionship & Daily Engagement: Supporting social interaction and overall well-being
Our CFSS program empowers clients and families to tailor care to their unique needs, promoting independence, comfort, and a better quality of life at home.
Stay safe and supported at home
We offer flexible in-home services so you or your loved one can stay safe and supported at home:
- Help with bathing, dressing, meals, and daily routines
- Support workers you choose and trust
- Personalized service plans tailored to your needs
Am I eligible?
You generally qualify for CFSS if all of these are true.
- You live in the community, not in a facility like a nursing home
- You can direct your own care, or you have a representative who can
- You’re enrolled in Medical Assistance (MA), an HCBS waiver, Alternative Care (AC), or certain MinnesotaCare plans
- You’ve had an assessment that determines eligibility
- You’ve selected the CFSS agency model

Every CFSS plan starts with an assessment. Who you contact first depends on how your care is paid for.

If you’re not enrolled in a seniors’ health plan, contact your county or tribal nation and ask for a CFSS assessment.

What is CFSS and am I eligible?
Community First Services and Supports is a Minnesota Medicaid program that lets you stay in your own home with help from workers you choose. You generally qualify if you live in the community rather than a facility, you can direct your own care or have a representative who can, you’re enrolled in Medical Assistance, a waiver, Alternative Care, or certain MinnesotaCare plans, you’ve had an assessment, and you’ve selected the agency model.
